There are disparities among the Valley communities, which categorically affect education, employment, health and well-being. The findings in the 2016 Valley Community Index provide the information needed to develop strategies for change.

Community conversations first took place in 2014 to discuss the need for a common-source report that would provide a comprehensive review of the challenges shaping the Valley communities. An advisory committee was formed comprising leaders from more than 30 local multi-disciplinary organizations
including: Griffin Hospital, area schools, municipalities, elected officials, a wide range of nonprofits, and volunteers. The Index was created in partnership with DataHaven, and fulfills Griffin Hospital's Community Health Needs Assessment, a requirement of all tax-exempt hospitals according to the Patient Protection and Affordable Care Act.
The Valley Community Foundation Board of Directors recognized the importance of building an informed community and took the lead in investing in this important initiative; other lead sponsors include: Bassett Family Fund; Griffin Health Services, Inc.; Katharine Matthies Foundation, Bank of America, N.A., Trustee; Valley United Way. Contributing Sponsors include Connecticut Community Foundation and Liberty Bank Foundation.
